doi:10.759/cureus.30401 Emphysematous cystis (EC) is a potentialy life-threatening urinary tract infection (UTI) characterized by the presence of gas within the blader wal and lumen. The extension of gas beyond the blader wal is rare and indicative of severe disease. We present a case of septic shock secondary to EC with the extension of air through the paraspinal and psoas muscles and into the epidural space of the lumbar spinal canal.

This finding of intraspinal air is a rare radiographic phenomenon known as pneumorhachis (PR).Emphysematous cystis (EC) is a rare type of complicated urinary tract infection (UTI) caused by an infection with gas-producing bacteria, most comonly Escherichia coli or Klebsiela pneumoniae [1]. The risk factors for EC include female gender, advanced age, uncontroled diabetes melitus, urinary stasis, recurent UTIs, imunosupresion, neurogenic blader, and indweling urinary catheters [2].
